Thanks to support from Chesterfield Health and Wellbeing Partnership, and our partner Primary Care Networks (Chesterfield Royal Primary Care, North-East Derbyshire, North Hardwick and Bolsover and South Hardwick), we are now looking to expand our team of link workers to provide even more community-based support for 14–18-year-olds (up to 25 with additional needs).
Employed by Chesterfield FC Community Trust, you will be working around the Chesterfield, North-East Derbyshire, and Bolsover areas. You’ll adopt a holistic, person-centred approach to your caseload, working with the young person to co-design plans to improve wellbeing and health. This will involve working with their individual strengths and needs, to support them to achieve their goals.
Ideally you will have worked in a supportive role with young people before and will have a great understanding of the many and complex challenges young people are currently experiencing.
The post is subject to an enhanced level DBS check.
